Benzodiazepine withdrawal symptoms, their intensity, and timeline vary significantly depending on which drug you use, how long you have been taking it, and your daily dose. Short-acting benzos like Xanax (alprazolam) produce faster-onset, more intense withdrawal than long-acting ones like Valium (diazepam). This guide covers the full timeline and what medical support actually looks like in practice.

Why Benzodiazepine Withdrawal Is Dangerous

Benzodiazepines work by enhancing the effect of GABA, the brain’s primary inhibitory neurotransmitter. They slow neurological activity. With regular use over weeks to months, the brain compensates by reducing its own GABA sensitivity and upregulating excitatory systems.

When benzodiazepines are removed suddenly, the brain’s excitatory systems have no counterbalance. The result is a state of neurological hyperexcitability that can produce seizures, severe anxiety, hallucinations, and cardiac instability. This is the same mechanism that makes alcohol withdrawal dangerous, since alcohol also works on the GABA system.

This danger applies not only to people who misuse benzos in high doses. It can affect people who have been taking a prescribed therapeutic dose for months or years. Duration of use matters at least as much as dose in determining withdrawal risk.

Xanax vs. Valium vs. Klonopin: Why the Drug Matters

The half-life of the benzodiazepine you have been taking significantly affects when withdrawal begins and how it progresses.

Short-Acting Benzodiazepines (Xanax, Ativan, Halcion)

Xanax (alprazolam) has a half-life of 6 to 12 hours. Withdrawal symptoms can begin as early as 6 to 12 hours after the last dose. The onset is rapid, symptoms peak within 1 to 4 days, and the acute phase typically resolves faster than long-acting benzo withdrawal, though it can be more intensely uncomfortable at peak.

Long-Acting Benzodiazepines (Valium, Klonopin, Librium)

Valium (diazepam) has a half-life of 20 to 100 hours, and its active metabolites can stay in the system for days. Withdrawal from Valium may not begin until 24 to 48 hours after the last dose. The onset is slower, but symptoms can persist for a longer period and the peak can be delayed by a week or more.

Klonopin (clonazepam) has a half-life of 20 to 50 hours. Withdrawal often begins 1 to 3 days after stopping and can be prolonged, with symptoms lasting weeks in some cases.

The Benzodiazepine Withdrawal Timeline

Short-Acting Benzos (Xanax, Ativan): Days 1 to 4

Hours 6 to 24: Early symptoms appear rapidly for short-acting benzos. Anxiety, agitation, and insomnia begin. Racing thoughts and muscle tension are common. Heart rate may increase.

Days 2 to 4: This is the peak danger window for short-acting benzo withdrawal. Seizure risk is highest. Symptoms include:

  • Severe anxiety and panic attacks
  • Tremors and muscle spasms
  • Sweating and heart palpitations
  • Nausea and vomiting
  • Insomnia
  • Heightened sensory sensitivity (light, sound, touch feel overwhelming)
  • Derealization and depersonalization (feeling disconnected from reality or yourself)
  • Seizures (in some patients)
  • Psychosis or hallucinations (in severe cases)

Long-Acting Benzos (Valium, Klonopin): Days 3 to 14

Days 1 to 3: Because of the longer half-life, symptoms emerge more slowly. Mild anxiety and sleep disruption begin, which may be mistaken for ordinary anxiety or insomnia rather than withdrawal.

Days 4 to 10: The acute phase for long-acting benzos. Anxiety, insomnia, tremors, and physical discomfort intensify. Seizure risk persists throughout this window. The slower onset does not make withdrawal safer overall; seizures can occur over a broader window compared to short-acting benzos.

Week 2 and Beyond: Symptoms begin to ease gradually but often do not resolve abruptly. Anxiety, sleep problems, and cognitive difficulty may persist for weeks.

Post-Acute Withdrawal Syndrome in Benzodiazepine Cessation

Post-acute withdrawal syndrome (PAWS) is particularly well-documented in benzodiazepine withdrawal. Some patients, particularly those who were on benzos for years, experience protracted symptoms that can last months. These include:

  • Persistent anxiety that does not respond to standard anti-anxiety strategies
  • Tinnitus (ringing in the ears)
  • Cognitive impairment, “brain fog,” and memory problems
  • Paresthesias (burning, tingling, or crawling skin sensations)
  • Sensitivity to light, sound, and touch
  • Poor sleep quality
  • Intermittent muscle spasms

Protracted benzo withdrawal is sometimes called “benzo belly” or benzo PAWS informally. These symptoms are real, they are physiological, and they gradually improve with sustained abstinence, though recovery can take six months to two or more years in some cases.

How Benzodiazepine Withdrawal Is Treated Safely

Supervised Taper

The standard medical approach to benzodiazepine withdrawal is a slow, supervised taper rather than abrupt cessation. A physician reduces the dose gradually over weeks or months, giving the brain time to recalibrate with each incremental reduction. The Ashton Manual, developed by British pharmacologist Dr. Heather Ashton, is a widely referenced protocol for benzodiazepine tapering, often using diazepam (Valium) as a substitute because of its long half-life and easy dose reduction in small increments.

A taper that is too fast is still dangerous. Many patients report that even a 5 to 10 percent dose reduction per month is the fastest rate they can tolerate without significant symptom resurgence. The right pace varies by individual.

Medical Detox

For people who are physically dependent on benzos and need to stop quickly, a medically supervised detox program provides round-the-clock monitoring and medication management. Anticonvulsant medications, supplemental benzodiazepines on a tapering schedule, and supportive care for sleep and anxiety can all be part of inpatient detox management.

What Does Not Work

Cold turkey benzodiazepine cessation is dangerous and should not be attempted. Over-the-counter supplements and natural remedies cannot prevent seizures. Alcohol as a substitute (both work on GABA) is a high-risk strategy that creates a new dependence while not adequately stabilizing the neurological state. If you are dependent on benzos, the safest path is through a medical provider, not around one.
